How to send us a referral

Send a prescription or order

Fax or send us a signed order with the patient's diagnosis, equipment type, and your NPI. We'll confirm receipt the same day.

We handle authorization

Our billing team contacts the patient's insurer, obtains prior authorization where required, and keeps your office updated on status.

Patient receives setup and support

We contact the patient, coordinate delivery, and provide clinical setup. You receive confirmation when equipment is in the home.

What patients and families ask us most

Does CrestCare Medical accept Medicare?
Yes. We are an enrolled Medicare durable medical equipment (DME) provider. Medicare Part B covers a wide range of home medical equipment — including CPAP machines, wheelchairs, and hospital beds — when ordered by a physician and medically necessary. We handle Medicare billing directly and will confirm your coverage before any equipment is provided.
What insurance plans do you accept?
We accept Medicare, Medicaid, and most major commercial insurance plans. When you contact us, we'll verify your specific plan and benefits before proceeding so there are no surprises. If your plan isn't one we're currently enrolled with, we'll let you know upfront and explain your options.
How do I get started if my doctor prescribed equipment?
Ask your physician's office to fax us the prescription along with any required clinical documentation. You can also call us directly — we'll contact your doctor's office to obtain the order and handle everything from there. Our team will reach out to you to confirm the details and schedule delivery.
Do you help with CPAP setup and ongoing supplies?
Yes. For CPAP and BiPAP therapy, we provide in-home setup and a clinical walkthrough so you understand how to use your device and mask correctly. We also run ongoing supply programs that automatically ship replacement masks, cushions, filters, and tubing on a schedule that aligns with your Medicare or insurance benefits.
What if I have a problem with my equipment after delivery?
Call us. We maintain a dedicated customer service line staffed by real people — not an automated system. For equipment issues, we troubleshoot over the phone first, and if needed, arrange for a replacement or in-home visit. We don't consider our job done at delivery.
Can family members coordinate care on behalf of a patient?
Absolutely. Many of our patients coordinate care through a spouse, adult child, or other family member. We're experienced at walking family caregivers through the process and making sure everyone who needs information has it. We just ask that you have the patient's authorization to discuss their health information with us.
Do you serve hospice and home health patients?
Yes. We work closely with hospice organizations and home health agencies as a preferred DME partner. We understand the documentation requirements and billing structures specific to hospice and are set up to respond quickly when a patient's needs change.
