Use these as a quick reference when preparing referral documentation. Send us what you have — we'll follow up with your office for anything outstanding.
Sleep Therapy (CPAP / BiPAP)
Required Documentation
- *Qualifying sleep study report (AHI ≥ 15, or ≥5 with symptoms)
- *Physician order specifying device type and settings
- *Face-to-face evaluation note within 6 months
- *ICD-10 code (e.g., G47.33 – Obstructive sleep apnea)
Helpful to Include
- ○Titration study results (for fixed-pressure CPAP)
- ○Prior CPAP history (if replacement)
- ○Insurance card or member ID
Home Oxygen Therapy
Required Documentation
- *Physician order with O₂ flow rate and hours/day
- *Qualifying oximetry (SpO₂ ≤ 88% at rest or on exertion)
- *CMN Form 484.03 (we send for your signature)
- *ICD-10 diagnosis codes
Helpful to Include
- ○Pulmonary function test results
- ○Arterial blood gas results if available
- ○Current medication list
Power Wheelchair / Scooter
Required Documentation
- *Face-to-face exam note within 6 months of order
- *Physician order specifying device type
- *Documentation of mobility limitation in the home
- *PT or OT evaluation (required for complex rehab tech)

- ○Prior authorization request (we submit on your behalf)
- ○Diagnosis codes supporting functional limitation
Hospital Bed & Pressure Mattress
Required Documentation
- *Physician order with bed type and rationale
- *Clinical documentation of medical necessity
- *CMN for powered pressure-relief systems
- *ICD-10 codes

- ○Wound care notes (for pressure mattress)
- ○Home health plan of care
- ○Discharge summary if post-acute
Manual Wheelchair
Required Documentation
- *Physician order specifying wheelchair type
- *Documentation that patient cannot ambulate in the home

What Your Team Can Expect From CrestCare
Confirmation Within 2 Hours
We confirm receipt of every referral within 2 hours during business hours and assign a patient coordinator. You'll receive the coordinator's direct contact information.
We Chase Documentation — Not You
If we're missing a CMN, lab result, or face-to-face note, we contact your office directly. We track outstanding documentation and follow up so your staff doesn't have to.
Delivery Status Reports
We send a delivery confirmation to the referring clinician or facility after equipment is in the patient's home, including what was delivered and any patient concerns noted.
30-Day Follow-Up
For CPAP and oxygen patients, we provide a 30-day compliance or usage summary upon request — supporting your clinical documentation and payer requirements.
Rapid Turnaround for Urgent Discharges
For hospital discharges, send us the referral and a target discharge date. We work backward from that date to ensure equipment is in the home before the patient arrives.
